The Student Health Inventory and Over the Counter Medication Form has now been combined. This form needs to be completed yearly for each of your children in the District as it provides important information for the school nurse and gives permission for your child to receive treatment and over the counter medication, if needed. Please take the time to accurately complete this form so the nursing staff has the information they need to properly care for your child while he/she is at school. Please contact the school nurse in the appropriate building to discuss your child's medical concerns.

If this form is not on file for the current school year, we will not be able to administer any over the counter medication to your child while they are at school.

As a general rule, we will not administer over the counter medication to students within one hour of school dismissal.
