Any high school or middle school student who plans to participate in school activities next year can receive a free physical on Thursday, May 18 from 5:00-7:00 p.m. in the high school commons.
Physical forms can be picked up at the HS or by clicking here (look for the Athletic Forms dropdown). Three forms must be filled out. All forms should be filled out and signed by parents prior to arrival.
A sign-up sheet will be available at the door on May 18 to register your student.
The Cox Mobile Unit will also be available to give the required immunizations for next year's 8th and 12th graders during the Free Physical Night on Thursday, May 18 from 5:00 - 7:00 p.m.
The Mobile Unit will be parked in front of the high school. You will need to bring a completed consent form and a copy of your child's immunization record.
