Food Allergies/Meal Modification Form

If your child will need a meal substitution due to a food allergy, a "Medical Statement For Student Requiring Meal Modification" form must be completed and returned to the building nurse .

The Food Service Department must follow governmental regulations which require the district to provide each student with the items on the menu for that meal, unless the meal modification form is completed and on file with the school.

 The form must be completed by a medical authority (MD, DO, Nurse Practitioner or Physician Assistant), and any modifications that need to be made for the student should be included on the form. A parent signature is also required.
