8th Graders Required to Receive Tdap Vaccination

Dear Parents:

 
The State of Missouri requires all students to receive one dose of the Tdap (tetanus,

diphtheria, and acellular pertussis) vaccine before entering the 8th grade. 

The law states it is the responsibility of the parents to provide documentation of the

month, day and the year the student received each required immunization or to provide a

completed Missouri Department of Health medical or religious exemption form to the

school.  Your child will not be allowed to attend 8th grade until you provide the

required information to the school nurse.
 

The State of Missouri does not allow either "In Progress" forms or a future appointment

for the Tdap immunization.  Therefore, it is important to schedule the immunization well

in advance of the due date so your student will not be excluded from school.

 
If you have private health insurance, immunizations need to be obtained from your

personal physician (fees vary). However, if you are uninsured, have MO HealthNet

(Medicaid) or underinsured, immunizations can be obtained from the Stone County

Health Department. A $15 fee will be charged for each student (no charge for those with

MO HealthNet). Appointments usually will be required, and students will need to have a

copy of their immunization record and be accompanied by a parent or guardian.

 
Insurance policies and regulations have changed since last school year, so, please

contact your doctor’s office with your insurance information/questions first, then you

may contact the school and I will do my best to help you with any questions you have.

 
To avoid a delay with your child attending class on the first day of school, please obtain

this required immunization promptly and provide the proper documentation to the school

as soon as the immunization is received.
