Staff from the Stone County Health Department will be available at Reeds Spring High School on Monday, May 13 at 5:00 p.m. during the free sports physical night to give the state-required immunizations to students entering the 8th and 12th grades.
Per State of Missouri law, students entering 8th grade are required to receive the Tdap and Meningitis vaccines, and students entering 12th grade are required to received an additional Meningitis vaccine.
There is a $15 fee for immunizations, payable to the Stone County Health Department. There is no fee if your child is insured under Mo Health Net (please provide copy of Mo Health Net card).
Parents will need to be present or send a dated and signed note giving the Stone County Health Department permission to give your child immunizations. You should also bring a copy of your child's immunization record.
